Remote membership form

Please read the following information carefully and click on the "Yes I agree" button below.

• This membership is for information access only (no borrowing).
• The membership will be valid for one calendar year from the day that you receive confirmation of your membership from the library.
• Please notify the library of any changes to your contact information. This will help us to keep in touch with you.
• All charges relating to the use of this library card must be paid before services will be actioned.
• Your library card must be presented to access library services.
• The library reserves the right to report any default of payment to a debt collection agency.
• You are entitled to access and have corrected if necessary, the information that we keep on your library record at any time.
• Please note that email communication may include Auckland City Libraries promotional material.
• I agree to follow the Auckland City Bylaws on library use.
• I understand and accept the conditions of membership and the failure to comply may result in cancellation of the membership.
• I agree that the information provided on this form is correct.
  

We will process your request for remote membership within two working days. If your application is accepted, you will receive confirmation of your membership, and your library card number via email. You will be able to use the library’s services from that day. Your library card will be posted out to you.

Note:

If you want to access the library’s services from within one of the libraries, you will need to bring your library card with you each time you visit. Your membership is for your personal use only and your library card should not be lent to anyone else. If you lose or mislay your library card, please notify us immediately so that we can prevent someone else from using it. You are responsible for the use of your library card unless you have reported its loss to us. Replacement cards cost $3.
